You could do it w/ Asterisk. You'd need a computer, one FXO box, and either
an IP phone, FXS box to use an analog phone, or you could use a soft phone.

Almost any old computer will run Asterisk, as it doesn't have to do much but
establish SIP connections. Asterisk@Home is fairly easy to set up.
If you're looking for a zero effort, drop in solution, this isn't it, but
you could get it up and running for under $300 ($20 FXO card, $60 for a
cheap IP phone, $200 for an old computer if you don't have one). Each
additional line would be about $80 (FXO card and IP phone). If you wanted a
bunch of lines, then you'll run out of PCI slots and will need to use
external FXO boxes which are a little more expensive ($80).
I don't know how technical you are, so this may or may not be an option.
You could probably hire someone to set it up for you as well.
