Arron,
I've terminated uncounted CAT5 drops and where 'premade' can be
inexpensive and work very well, the Netday installations I've
participated in have yielded 100% testing for 10/100 operation as we
completed them. There are no real secrets to the premade ones ...
although they do follow industry standards.
Custom cables require 4 things: good wire (anything with a CAT5 rating
MUST meet minimums ... 10/100 is not a stretch up to 100 meters), the
proper RJ-45 connectors solid or stranded for same cable, a good crimp
tool, and good technique.
However, where I think technique is very important ... even sloppy
technique wouldn't really explain your problems easily ... and I think
there is another answer.
